TRICARE Active Duty Dental Program (ADDP) Information for Ordering Materials

The following materials are available to order via our Web site online form. Simply fill out the form, click on the Mail Form button, and your order will be filled within the next business day. An explanation of the item, its use, and if there is a limitation as to the number that can be ordered is beside each item below.

ADDP DTF-referred Flyer This flyer is only available for military dental treatment facilities (DTFs). It should be given to active duty service members (ADSMs) who have received a DTF referral for private sector dental care. The flyer provides specific ADDP guidance for the ADSM upon DTF referral.
ADDP Remote Brochure This brochure provides detailed ADDP information and process instruction for remote ADSMs (live and work 50+ miles from a military DTF).
