Who is eligible:

The ADDP is available to active duty service members (ADSMs) of the:

  • U.S. Air Force
  • U.S. Army
  • U.S. Coast Guard
  • U.S. Marine Corps
  • U.S. Navy
  • National Oceanic and Atmospheric Administration (NOAA)*
  • National Guard and Reserve Component Sponsors (called or ordered to active duty for more than 30 consecutive days).
  • *NOAA members are eligible for the remote ADDP

The following are also eligible:

  • National Guard and Reserve "Early Activators:"
    • Some members of the National Guard and Reserve who are issued delayed-effective-date active duty orders for more than 30 days in support of a contingency operation or a preplanned mission, are eligible for "early" TRICARE medical and dental benefits beginning on either: (a) the date their orders were issued or (b) up to 180 days before they report to active duty.
    • National Guard members moved from federal orders to state orders while performing state disaster response duty as if on active duty orders for more than 30 days
    • These members are eligible for the same dental services provided to active duty service members. They are eligible to receive civilian dental care through the remote ADDP.
    • National Guard and Reserve members may call United Concordia's ADDP Department at 1-866-984-ADDP (2337) to verify their ADDP eligibility if they have received delayed-effective-date active duty orders.

      If the service member has a non-eligible status in DEERS, he/she is not eligible for ADDP dental care. It will be the Guard Unit's responsibility to update the correct status in DEERS.
    • National Guard and Reserve members may also verify their eligibility for TRICARE through the secure Guard and Reserve Web Portal website at https://dmdc.osd.mil/esgr/logonAction.do (if the employer input page appears, click the "BACK" button located on the bottom of the "employer input page"). For assistance with an eligibility problem, members should contact their Service Point of Contact listed on the Defense Health Agency (DHA) website at www.tricare.mil/earlyeligibility.

  • Personnel in the Transition Assistance Management Program (TAMP) following activation for a contingency operation for more than 30 days are eligible to receive private sector dental care. These members are eligible to receive civilian dental care through the remote ADDP for up to 180 days during the TAMP period; however, dental implants, orthodontics, and extensive treatment plans are not covered. The 180 days does not allow sufficient time to complete extensive treatment plans and partial treatment plans will not be approved.
  • Wounded Warriors
    • Wounded Warriors are service members or veterans who have a disability that either resulted from injury or disease received in the line of duty as a direct result of armed conflict, or was caused by an instrumentality of war and was incurred in the line of duty during a period of war. Wounded Warriors are identified in the Defense Eligibility Enrollment Reporting System (DEERS).
    • ADSMs identified as Wounded Warriors in DEERS are eligible for private sector care under the ADDP. These ADSMs should contact UCCI at 1-866-984, ADDP (2337) to coordinate their treatment prior to receipt of care.
    • ADSMs identified as Wounded Warriors in DEERS who are receiving inpatient or outpatient care at a VA Hospital are eligible for the ADDP.
  • Line of Duty (LOD) Service Members:
    • LOD service members are only eligible for DTF and government-paid private sector dental care in cases where injury or illness occurs during active duty status and requires dental treatment.* ADSMs must have a LOD determination from their Service that is signed by their commander or designated representative and are only eligible for treatment for that injury or illness. Documentation for LOD eligibility verification must be sent to United Concordia via fax (1-866-308-4138),  email (addpdcf@ucci.com) or mail by the ADSM, military DTF or Government representative. ADSM contact information must be included.

      NOTE: Submitting LOD documentation is NOT authorization to receive private sector dental care. The Government will review the LOD paperwork and United Concordia will notify ADSMs of the determination.

      *Care for National Guard and Reserve remote service members on orders for 30 days or less: LOD documentation must be reviewed and authorized by the Defense Health Agency (DHA) (via LOD documentation submission through United Concordia) prior to receipt of any private sector dental care.

      If the ADSM has a dental emergency and feels he/she cannot wait for the Government review and authorization, the decision to receive treatment for that dental care may put the service member at financial risk. If the Government review of the LOD case determines the care was not a dental emergency or does not fit the requirements of an LOD, the service member will be responsible for all charges associated with that care.

      No definitive treatment may be received without LOD documentation and subsequent Government approval. Non-remote National Guard and Reserve service members must seek care from a military dental treatment facility.

    • Service members receiving treatment for line of duty conditions will not show as eligible for care in the Defense Enrollment Eligibility Reporting System (DEERS). To verify eligibility, LOD service members can contact their unit administrator or medical representative.
  • Foreign military service members stationed in the United States
    • Foreign military personnel must show as eligible in DEERS in order to access care under the ADDP. This means the service member's country must have a reciprocal agreement with the United States that authorizes dental care.
    • In instances when DEERS does not note eligibility, the foreign service member or referring DTF must fax invitational travel orders to United Concordia at 1-717-433-9864 prior to making a dental appointment and receipt of private sector dental care. United Concordia will forward the orders to the Government for a final eligibility determination. Private sector dental care is not authorized without Government approval. United Concordia will inform the member and/or referring DTF of the determination.
