ADDP Claim Submission Frequently Asked Questions

Dentists must indicate the ADSM's dental readiness classification code on the claim form, as this allows the military to track the ADSM's readiness for worldwide deployment.

  1. Where do I send ADDP claims?

    There are three available methods to submit your claims:

    DART®+: Search for an exisiting authorization or referral in DART®+ via the Search function. Click on the referral or authorization number you want to submit as an eclaim. Then click ‘Prepare eClaim.’You will need to enter the requested provider information (United Concordia Provider ID, Tax ID/SSN, and/or NPI) in the ‘Billing Provider’ section and click ‘Retrieve Billing Provider.’ You will then need to complete the ‘Claim Information’ and ‘Services’ sections. Please be sure to include the DRC and Service Date (procedure information from the authorization/referral will carry over into this section). You will also need to enter the requested provider information into the ‘Performing Provider’ section. Finally, click ‘Submit eClaim’ (this will open a new window into Speed eClaim®). All information that was entered into the ‘Prepare eClaim’ section will carry over into Speed eClaim®. Click Submit.
      Please note: if you want to submit another claim for a different ADSM, close the Speed eClaim® window and go back to the search screen in DART®+ (and then repeat the above steps).

    To ensure accurate and timely processing of claims, the ACN (provided by remote Active Duty Service Member or noted on the Referral Form) and the Dental Readiness Classification must be indicated in the appropriate area of the claim form. The ADDP claim form is available online at www.addp-ucci.com. Claims may be submitted by mail to:

    United Concordia strongly suggests you submit claims electronically. Electronically submitted claims are processed faster than paper claims, which means faster reimbursement to you.

  2. How long do I have to submit a claim?

    Your completed claim must be submitted within 60 days of the rendered service. However, we recommend you submit the claim form as soon as possible. Claims submitted more than 12 months after the month in which the service was provided will be denied.

  3. I submitted a claim for my patient and received a letter requesting my license and tax identification. Why is this information required?

    Contractually, United Concordia is required by the Federal Government to verify licensure and tax identification of all providers rendering services covered under the Active Duty Dental Program.

  4. My patient's claim has rejected due to incomplete data. What do I do now?

    Submit a new claim with the required information.

  5. My patient's claim was rejected for missing information. The information was reported on the original claim. What do I do now?

    If the information was reported on the original claim, call ADDP Customer Service at (888) 286-8454 and verify that all the necessary information was received. If additional information is still needed, resubmit the original claim with the required information to the claims processing department.

  6. When is it necessary to submit X-rays?

    Diagnostic requirements are available on the Reference Materials page of our Website. From this page, click Diagnostic Requirements to the left, under ADDP Program Overview. Note that if you submit original X-rays, United Concordia will return them only if a self-addresed, stamped envelope is included with the authorization.

  7. What if my claim was missing required information?

    The claim will be denied and the Dental Explanation of Benefits (DEOB) will indicate what information was not reported on the claim submission. The claim can be resubmitted with all required information or you may contact customer service at (888) 286-8454 to report the missing information.

  8. How can I get a claim reviewed?

    If you do not agree with United Concordia's determination of benefits, you may request a review by contacting ADDP Customer Service at (888) 286-8454.
