Find a Dentist

IMPORTANT

Although this Find a Dentist feature is available to help you locate a United Concordia network dentist, active duty service members (ADSMs) MUST receive an Appointment Control Number (ACN) prior to receipt of private sector dental care under the Active Duty Dental Program (ADDP).

ADSMs who receive private sector dental care without an ACN will be responsible for all charges related to that care.

To make an appointment under the ADDP, select the Make an Appointment tab in the ADSM portal on the home page. You can then select from the DTF-referred or Remote options.*

*DTF-referred includes ADSMs who reside or work within 50 miles of a military dental treatment facility (DTF). Remote ADSMs reside and work greater than 50 miles from a military DTF.
